The Cathedral of Pain is a trial for heroes, villains and Praetorians level 40 and above. It can only be started via the Team-Up Teleporter menu.
Description
Trial: Cathedral of Pain
Required Number of Players: 12 to 24
Objectives: Release the Item of Power.
Rularuu stores his Items of Power in extra-dimensional vaults he calls his Cathedrals of Pain. Each Cathedral is shielded by three obelisks called Psionic Resonators, which are in turn protected by horrors spawned from a nearby Will Forge. Only when the Will Forge spawns are defeated can you attack the resonator. Destroy all three at once and the portal to the Cathedral of Pain will open wide!
Requirements
The Trial accommodates up to 24 players of level 40 and up, which can form three teams.
All players are sidekicked/lackeyed to 50 regardless of team leader levels.
Trial Details
Destroy All Obelisks
You must defeat the nearby guardians spawned by the Willforge. Once these creatures are dealt with, a protective field around the nearby Polypsionic Resonator will drop, leaving it vulnerable to attack. If all three resonators are destroyed within 35 seconds of each other, the portal to the Cathedral of Pain will open.

Players arrive at a crystal on Alpha Isle in the northwest of The Courtyard of Shadows. There are three beams shooting up into the air in the distance. The three beam emitters are called Psionic Resonators and are marked on the mini-map as Psionic Resonator of Woe on the northeast Beta Isle, Psionic Resonator of Despair on the southeast Gamma Isle, and Psionic Resonator of Gloom in the southwest Delta Isle. These resonators are protected by a shield. Near to each resonator there is a cube-like object called a Willforge floating in mid-air. It spawns large groups of Soldiers of Rularuu. In order to destroy a resonator, defeat all guardian Rularuu that are spawned from the nearby Willforge. Once all the Rularuu are defeated, the shield will drop and you can damage the resonator. All three Psionic Resonators must be destroyed within 35 seconds of each other, so the teams must coordinate their destruction. Once you bring them all down, the door to the Cathedral of Pain opens via the green crystal on the Portal Isle in the centre of the map.

Release the Item of Power
You must again battle the spawns of the Willforge. When they are defeated, you can attack the Aspect of Rularuu that guards the treasure you seek.
- Aspect of Rularuu - 100/100
After entering the Cathedral, characters can look upon an Aspect of Rularuu. He is surrounded by a shield similar to the ones around the Resonators, and within the Cathedral are three more Willforges. The shield can be brought down by defeating all three groups of Soldiers of Rularuu. Once all the mobs are defeated, characters have about a minute to fight the Archvillain before the Willforges summon more enemies, bringing the shield back up. He has good regeneration so Temporary powers such as Shivans, and other strong debuffs can improve the chance to defeat the Archvillain in one attempt. (Note: Warburg nukes cannot be used during this trial.)
Rewards
Successful completion of the Cathedral of Pain Trial awards the Pain Killer badge.
You've completed the Cathedral of Pain and thwarted the Aspect of Rularuu's plans.
Additionally, participants have three reward options to choose from:

- Item of Power - Awards one of the below Temporary Buff Powers, 11 (Reduced to 5 if repeated within 18 hours on the same character) Reward Merits and the Power Liberator/Master Thief badge. These temporary powers are mutually exlusive:
Your success at liberating items from trophy vaults has earned you the title of Power Liberator.
There isn't a trophy vault made you can't steal from!
Auto: Enhances Damage
Viewing the Anger Monument filled you with rage and power. Your damage of all types is increased. Duration: 7 days (Real time)
Auto: +Max HP, +Regen
You have released the still beating Heart of Daralak, a mighty brute. The Heart increases the health and regeneration of those exposed to it's unique energies. Duration: 7 days (Real time)
Auto: Resist(Hold, Sleep, Immob, Stun)
Exposure to the Crystal of Resilience has given you a slight resistance to certain mind-affecting powers. Duration: 7 days (Real time)
Auto: +Perception, +ACC
You have gazed into the unblinking guardian of Rularuu's cathedral and gained power from it's influence. Your gaze pierces darkness and even dimensions. Your Stealth Detection and Accuracy are both increased. Duration: 7 days (Real time)
Auto: +Max End, Res(End Drain)
You have bathed in the heat of the True Furnace which Rularuu used to construct his inter-dimensional factory cubes. The True Furnace radiates energy, which has greatly increased your endurance reserves. Duration: 7 days (Real time)
