The Minds of Mayhem Trial (M.o.M. or MoM) is one of the Incarnate Trials connected to the release of Issue 21.5 which takes place in Praetoria. A minimum of 8 and a maximum of 16 players can raid the Mother of Mercy Psychiatric Hospital's grounds.
During the mission, a voice will "radio in" information about your progress. While there is no specific contact (the trial is entered entirely through the LFG menu), it is possible that your unseen helper is meant to be Prometheus as he was added in Issue 20 to recruit Incarnate-capable players to wage a war against Emperor Cole and end his quest to become more in-tune with the Well of the Furies.
All Issue 20 Incarnate Components will drop during this trial, and Psychic Incarnate Experience is gained towards unlocking the Alpha, Judgement and Lore Incarnate Slots.
Story Summary

In Detail
The Incarnates take the battle to defeat Emperor Cole's regime deep inside the mind of madness against Malaise and Praetor Tilman at the Mother of Mercy Psychiatric Hospital. The fight is split into five distinct phases.
Phase 1 - Malaise
The first phase is against Malaise, who assaults the Incarnates with Nightmares of various Giant Monsters from throughout the game's story.

Phase 2 - Penelope Mayhem
After Malaise is defeated, Penelope Mayhem assaults the league with psychic storms, phasing in and out of immunity to damage.
Phase 3 - Mother Mayhem
After Penelope Mayhem is defeated, the Incarnates are faced with the threat of Mother Mayhem, while also needing to protect the essence of Aurora Borealis.
After rescuing Aurora from the mind of Mother:
Aurora Borealis: Tilman is still a threat. If we can't disconnect her, she can just control someone else. We'll have to...
Aurora Borealis: Wife?! Who do you think you are?
Calvin Scott: It's me, Calvin! You... don't remember your own husband?!
Aurora Borealis: Husband? I don't have a-
Aurora Borealis: Wait... Calvin? Calvin Scott?! My patient from the asylum? I almost didn't recognize you.
Calvin Scott: No! Tilman did something to your mind... She's made you forget me! She's made you forget US!
Aurora Borealis: Mr. Scott, please. Shalice Tilman has done much, but I remember you, and we are not married.Calvin Scott: No.
Aurora Borealis: Schizophrenic, yes. I treated his depression for years. He had lost everyone in the wars, and grew attached to me... He must have fabricated that I was his wife when Tilman betrayed my trust.
Calvin Scott: No!
Calvin Scott: This can't be!
Calvin Scott: That isn't true! That can't be...
Calvin Scott: That would make everything I've fought for... Everything I've done...
Calvin Scott: NO! It's not a lie! It's... not... it can't be... I can't be... I'm not crazy!
Phase 4 - Mother Malaise
This phase is a fight against Mother Malaise -- a reprise of the fight against Malaise and all of his Nightmares, as well as Mother Mayhem. Both AVs must be defeated three times each.
Phase 5 - Shalice Tilman
With no where else to hide, the final phase is directly against Shalice Tilman.

Experience
- Psychic Incarnate Experience is earned, which unlocks the Alpha, Judgement and Lore Slots, if any of those slots are locked.
Salvage
- Incarnate Threads drop instead of Incarnate Shards.
- Upon successful completion of this trial, each character is presented with a reward table allowing them to choose one reward from several options. There is a participation threshold that must be met in order to receive an Incarnate Component table instead of a 10 Threads or one Super-level inspiration "consolation prize".
- If a character surpasses this threshold, the rarity of the component reward is a random roll. The options available will either be a choice of: one Common component or two Super-level inspirations, one Uncommon component or three Super-level inspirations, one Rare component or four Super-level inspirations, or one Very Rare component or five Super-level inspirations.
- The chances of receiving a Rare or Very Rare Incarnate Component upon completion of this trial are higher than in other Incarnate Trials.
- A random Uncommon Incarnate Component is awarded the first time each achievement badge is qualified for.
- A random Rare Incarnate Component is awarded the first time the Master of Minds of Mayhem badge is qualified for.
Note: The benefit of earning Dreamwalker/Dreamkiller can only be earned once per character ever.
Merits
- Two Empyrean Merits will be awarded upon successful completion of the Trial once per 20 hour period.
- Any successive completion of the Trial within that 20 hour window will instead award one Empyrean Merit. This functionality is essentially the Incarnate Trial version of Merit Reward Diminishing Returns.
- An Astral Merit will be awarded immediately after defeating Malaise in phase 1.
- An Astral Merit will be awarded immediately after defeating Penelope Mayhem in phase 2.
- An Astral Merit will be awarded immediately after completing phase 3's objectives.
- An Astral Merit will be awarded immediately after completing phase 4's objectives.
- An additional Astral Merit can be rewarded at the end of the trial for running as an Open League.
- An additional Astral Merit can be earned if the requirements for A Perfect Storm badge are fulfilled, even if you already have the badge. It is awarded at the end of the trial.
- An additional Astral Merit can be earned if the requirements for the Daylight Saver badge are fulfilled, even if you already have the badge. It is awarded at the end of the trial.
- An additional Astral Merit can be earned if the requirements for the Lost Connection to Server badge are fulfilled, even if you already have the badge. It is awarded at the end of the trial.
